INFORMATION & INSTRUCTIONS

TO: ALL LIONS CLUBS IN GEORGIA
Each Award applied for must have supporting information attached when so required. DO NOT use information on one application to apply on another application. When same information applies to more than one application, use copies and attach to each application. NO VIDEO TAPES, CASSETTE TAPES, CD’S, OR OTHER RECORDING DEVICES will be allowed in support of applications. Electronically submitted MMR’s must have a copy kept at the club level to be used when applying for state awards if you do not do it at this time.

We urge that you become familiar with rules governing each Award you apply for and follow them closely – so that the Committee can judge intelligently.

A combination of two or more Clubs jointly applying for an award WILL NOT be considered. The State Awards Committee has been instructed to disqualify any such entry. Lioness Clubs and/or Branch Clubs may enter all Lions Club awards that they are eligible to enter.

Recap all entries on the attached sheet. All applications, with supporting papers, must be attached to this Recap Sheet. (Fill in your Club name, date, and have sheet properly signed by a Club Officer.)

All applications for Awards (with supporting papers attached) MUST BE IN THE HANDS OF YOUR DISTRICT’S COMMITTEE MEMBER (LISTED BELOW) BY MAY 20, 2012. (5 days before the 2012 Georgia Lions State Convention opens). Committee Members’ names, addresses and telephone numbers are listed below:
